Acted as the Product Manager for the transformation project, overseeing product discovery, UX design, architecture, engineering, training, and the global release to 10 markets.

ML Pilot Implementation of RAF Score Estimation

Business Context

CMS uses Hierarchical Condition Category(HCC) method to calculate risk scored for Medicare Advantage patients. Higher category risk scores represent higher healthcare costs. Missed HCC groups in patient’s problem list result in a reduced insurance payoff.

Execution Approach

A neural network analyzed chest x-rays of the patients and predicts the scores for each HCC groups.

The predictions are being compared with up-to-date risk groups to find cases where HCC assignment was missed with a high confidence.

Patients with the highest probabilities are send to review by the doctor to further improve the model’s performance.

Role

Lead the 3 member team of Data Scientist, ML Engineer and Healthcare SME(Doctor) to build a prototype to predict the RAF score based on X-Ray images.

Outcomes & Impact

Successfully delivered the program that calculated the potential business value for the clinics.

Based on the expected precision of 80%, an estimated annual savings of $2.4M was calculated.

Clinic of the Future Implementation

The client wanted to accelerate the development of it’s future, omni-channel care model, which will completely reimagine distinctive, user-centered health and care experience for Medicare Advantage Patients and their Provider Community.

Execution Approach

The solution required the creation of a new service design that would enable their healthcare providers to drive patient engagement through optimized experiences and facilitated patient health literacy.

A digital platform enables clinics and their supporting call centers to maximize outcomes through optimized scheduling, personalized primary care and collaborative primary care journeys and effective outreach to engage with the patients.

Role

Led a multi-million dollar program to lead the team of Consultants to create the service design for the new Clinics.

Acted as a product manager briefly and created the roadmap to implement the Digital front door for patient access, scheduling, integration with EHRs, Omni-channel outreach using Twilio.

Led the implementation teams to deliver on the overall solution architecture, MVP, and Enhancements to develop a scalable cloud-native application. Implemented several integrations with EHR, and Remote Patient Monitoring devices.

Outcomes & Impact

Successfully delivered the program that changed the Providers way of working and helped them engage with clients better.
